I just wanted to raise what I consider to be an important point following a visit today the unit. I couldn't fault the way in which I was treated by all of the staff on duty. They were efficient, courteous and responsive. My issue is that when giving my details on reporting to reception I was asked who I wanted to put down as my next of kin. In my case I requested this be my civil partner. I was advised there wasn't such a box on the details form to tick. Civil Partnerships have been around now for over 7 years and I find it hard to believe that the NHS, the largest public service in the UK and one which probably has a very well written equality and diversity statement fails to recognise my relationship status, This may seem a trivial issue but on any discussions about treatment in the event I was unable to give consent or have the capacity to make decisions about my treatment it would be my civil partner and not my family who I would want to make decisions for me. Can an additional simple box for ticking "civil partner" please be added. Thankyou
