"COPD: Early Supported Discharge/Admission..."

I would like to say a big thanks to the ESD/AAS team for everything. I've been admitted via A&E twice recently with pneumonia, community acquired, I hasten to add. On both occasions the ESD/AA team took over the management of my case; liaising with Consultants, Nurses, and Pharmacy. I was sent home after 3 nights stay into their care. This was great: usually it would be at least a weeks stay. I was visited at home by the Team until they discharged me. I must congratulate Mid Yorkshire Hospitals on implementing these schemes: they must save the trust thousands. And for me as a patient going home early is wonderful. But, without a doubt the members of the team are the stars. Every one highly skilled and experienced Respiratory personnel but dedicated and professional.
