"After-care prior to being discharged"

Before being discharged I was offered pain killers to take home but no laxatives. As a result I became so constipated that I was impacted. I had to call out the emergency team. When the constipation was releived by a powerful laxative it was found that I had developed a water infection plus what looked to the doctor like a skin infection around the site of the wound. Before the operation I had been taking Warfarin and this was stopped 5 days prior to the operation but I was not told by the hospital to double the dosage when I arrived home. I later found out that this doubling is the normal thing to do. There is supposed to be some overlap in after-care between what the local services provide and the hospital provides. I didn't have any.
