"Oral Sugery -Plane Tree Ward "

I was booked in for an appointment to operate on day in early September 2014 at 7: 30am.

When I arrived to the Plane Tree Centre I was seen to by a nurse called Joan she carried out the usual procedure of taking my temperature, pulse and heart rate, she then asked me to get changed into the gown. I was told by another Lady named Laura that I was first on the list to be operated on. I waited some time for anybody to come back to me and inform me that my notes were lost. A couple of hours later the young junior doctor and Joan had both told me they could not find my notes anywhere, by this point it was around half past twelve.

They had asked me if I wanted them to have another look to see where they could be, I waited another half an hour until anybody came back to me. The young junior doctor told me he was unable to find my notes and that they would have to postpone my surgery, but did prescribe me Co-Amoxiclan to help relieve the pain.

I asked the doctor for the complaints department number as I was disgusted how my notes had been lost and I had been put through inconvenience and anxiety. I also asked to speak to a Service Manager to log my complaint and nobody in the department even knew what a service manager was, or who they might be.

As well as the inconvenience of today, I have had to take unpaid sick leave for the next seven days from work. Then there is the time wasted at the hospital on the day of the operation, waiting to find out my notes had been lost. I have also paid out £9. 00 for car parking today.

This is the second occasion that I am having this operation, as the first one was unsuccessful, not only have a had anxiety of the first failure I have also experienced further anxiety through not having my operation today and the loss of my notes.

I am now left with no confirmed date for my operation or structure in place going forward with this situation.
