"Consultant has not got in touch with me"

I am getting increasingly frustrated with your lack of communication. I have sent you two emails regarding my recent test results and symptoms and you do not reply to me. I have now written to my GP requesting a transfer to a different consultant. Your disregard for my care is just completely unacceptable. I have symptoms that need answering. And if you cannot be bothered to help me then I will find someone else who will.

Unfortunately, without your details I cannot progress the matter.
