About: Telford referral and quality service (TRAQS)

(as the patient),

Succession to TRAQS letter, appointment to see consultant at Donnington arrive within 2 weeks, (Mid August).

Good consultation, consultant agreed plan proposed by GP Dr Fairbanks.

To arrange further consultation in 6 weeks.
