About: Telford referral and quality service (TRAQS)

(as the patient),

Visited GP with sub-acute lupus outbreak (previously treated two years ago at the Princess Royal Hospital) - referred to DR Hart at Donnington who knew of my previous situation as she treated me at the Princess Royal Hospital and is very caring and professional, I have every confidence in her.
