"Concerned about documentation"

I have been to two separate clinics at Charing Cross Hospital and one has been fantastic, but I am concerned about my treatment from the breast clinic. My appointment was the last of the day and the staff were friendly- it was my first time to a breast clinic. I feel my symptoms were not all addressed and this was confirmed in the GP letter I received a week later. Three individual aspects of my symptoms were incorrectly documented. One sentence claimed I had had a lab test that was all normal, despite the fact that I had not had this test, and I have reason to believe that the result would not be normal. It has led me to distrust the result of this appointment. When I called the clinic to question this, the receptionist offered to edit the letter, removing these facts. A request for a second appointment was rejected and the telephonist demonstrated lack of understanding or care. I will not recommend this clinic to others.
