"A and e and acute dependency unit"

Relative was discharged with someone else's discharge letter and when's peaking to sister over phone she advised me it was just a human error. This could have caused a lot of damage. Also A&E one member of staff was very rude and was talking in their own language to the other member of staff. On arrival to M16 in A&E there was a bed pan full of urine. Shocking experience and will be writing to cqc. Also spoken to pals.
