(as a relative),

My husband was admitted 3 weeks ago with dementia. He wasn't eating or drinking. The nurses and other staff tried to tempt him to eat but without success. Last Monday he was transferred to a side room and myself and family were able to stay with him when ever we were able. I stayed with him for four days only going home for a shower and a change of clothes. The staff were all very kind and felt I had many friends. Sadly he passed away shortly after 11pm last Thursday.

Went to the hospital today to pick up my husbands certificate and register his death. Being able to do both at the same time was made very easy as it is a process most people feel very uneasy about. Thank you.
