Following an appointment with consultant in Urology dept on June 2014 at 3.55 pm, medication was prescribed to be obtained from the hospital pharmacy. Tamsulosin capsules were prescribed and when this prescription was dispensed an ear spray was issued. This error was not discovered until I returned home, so a return trip was needed to get the correct medication the following day.
