"Lack of communication"

About: Wexham Park Hospital

Reviewed hospital a week ago. Moved to a better ward [3] and what a difference. Told that a conference call was to be held on Monday about my care and discharge. Husband told Monday morning discharge was to be to home asap with necessary equipment ordered for urgent delivery. Husband waited in all day for the equipment, did not arrive despite promises. Results of telephone conference not shared. Tuesday, husband visited p.m., told that discharge to home had never been agreed, the equipment had been cancelled and transfer to community hospital arranged for next day. This had been agreed at the conference call on Monday ! But nobody told me or husband. No reason given for initial refusal to send to rehab two weeks ago, just glad to be on mend now - I hope. No apology from the OT team for not letting me know anything, or husband, just left to the equipment supplier to investigate and apologise.
