"Forgotten about"

My son had an appointment with the paediatric dietician in November 2013 who wanted to have us referred for my 3 year old to have an allergy test against vitamin. Went to the appointment yesterday to have this done an was told that this test doesn't actually exist and they wanted him to have a test to see if he is allergic to Penicillin because he had a reaction 3 years ago. The doctor also pointed out that te dietician left no notes in my son's record book and they have no idea who we saw. Theres nothing that dates back to that appointment which is very scary.
