"Transition from Children to Adult services"

My last appointment with the paediatric consultant was 1 or 2 years ago and in Leicester, LRI (about 45 minute drive from my house). He said that because I don't really need or use the NHS services, it was a bit far to travel for a 30 minute 'hello, goodbye' appointment and I was nearly 18, to just go to my GP if there was a problem and they'll refer me if necessary.

I wasn't introduced to any consultants or nurses within adult services which is concerning me because I don't know what notes will be passed on or whether I'd have to start my story from scratch.

I say that there needs to be more of a clear and understanding transition for people by explaining what happens if you're referred and the differences between children and adult services, especially if they've got a disability like me!

Thank you for taking the time to post your comments. It is hard to respond with exact advice/details as we do not know your history, personal details and condition etc from your comment above. However, we wanted to reassure you about a couple of things you mention.

Firstly, it sounds like your paediatric consultant has discharged you from his care back to your GP, rather than referring you straight onto the adult service at Leicester's Hospitals. If you had been transferred directly to the adult service, there is a consultant-to-consultant referral process which would have involved an opportunity for you to meet your new adult consultant.

As explained, it seems that you have been discharged to your GP, so in this case, if you do require further treatment, your GP will arrange a new referral to the adult service for you.

Although you are no longer seeing one of our consultants, your notes are kept with the hospital and will be accessible to any member of staff/service who treats you in the future. If you do come in and see adult services, you may need to have an initial chat and recap about your condition, but your full history will be available to them through your notes.

I hope that offers some reassurance and we are very sorry that this process wasn’t clearly explained to you at the time. If you have any further questions, I’m sure your GP will be able to explain how things will work for you from here on out,
