Someone who inspects patients records seems to be deleting/changing prescription items without consulting the patient. Two of my Stoma items have been deleted & one has had the quantity changed to a meaningless quantity. (19 years with a Stoma)
One medical item that I will not need again has not been removed.
Two items have also been removed from my wife's record without consulting her.
