About: Queen's Hospital, Burton Upon Trent / Trauma and orthopaedics Telford referral and quality service (TRAQS)

(as the patient),

Burton-on-Trent A&E, two day stay due to dislocated shoulder. Excellent care/service!

Referred to GP nearer to home. Dr Fitzgerald Frazer excellent as usual, referred to Newport Cottage Care ‘physio’.

Contact letter received with this form. Message left with my son to make an appointment. Called in Feb, made an appointment for further review.

NHS physiotherapist department Shropshire Community Health follow up letter dated Feb reminder – wrong name!
