"DRI - Eye Clinic - record mix up at Doncaster"

I have had excellent care in this department, especially from Mr Jayamanne. But in 5th Dec 2006 the person I saw (not a consultant) got my records mixed up and all my details went on another patients' file, also another appointment was made for me with the other patients name on it. I went back to get this all sorted out, everyone seemed to be in a panic and the person who I had seen came out of his room to look over at the files, he walked past me several times but not once did he apologise to me for his mistake. A nurse then took me into a room and my paper was taken out of this ladies file and placed into my file. What would have happened if I hadn't have noticed this?
