Out dating systems which will only register certain tick box answers are not appropriate, I have had a situation occur that should never have happened, which has caused lack of trust and worry about care. I also have had to wait 6years for treatment and it still not insight, this puts patients lives on hold, this service needs major help. The staff work with what they are given, but can only do so much. These and other problems have also resulted in The health minister, Ben bradshaw mp, and others being informed. Action is needed here.
