About: Chorley & South Ribble Hospital

The care is haphazard and uncoordinated. The medical teams don't pass on the relevant information to each other and the follow up care and communication with the GP appears to be non existent. The care is based on symptom diagnosis and treatment and not looking at what is causing the symptoms. Once the symptom has been treated you are discharged. A fact which caused my readmission some days later with the same cycle of symptomatic treatment and non communication.

Thank you for providing feedback about the care and treatment we provide. We do have robust procedures to ensure information is passed between teams during handovers, and to the patient's GP or other health or social care agency where necessary. We are sorry you feel this hasn't worked in your case. If you would like to provide further details, or would like a personal response, please email customer.care@lthtr.nhs.uk, or you can speak with our PALS team on 01772 522972. We are committed to continuously reviewing and improving how we work to make sure all of our patients have a good experience. Best wishes.
