"Discharge Liason"

About: Countess Of Chester Hospital

My Father was recently diagnosed with gastric cancer and pallative care was recommended. With little consultation,(with the family), discharge liason sent him for rehabilitation and assessment, against both the families wishes and his consultants advice, We didn't want him to moved twice, given his condition and his likely prognosis. We stated ,which care home we wanted him sent too and made an application independently. I found the attitude of the discharge liason team, both dictatorial and unhelpful, so much so, that I have asked for information to launch a formal appeal.
