"Blood Results lost. 3 times in a row"

My wife was 34 weeks pregnant and her blood results were lost for the first time. Upon inquiry, the doctors and nurse said that the nurse who took the sample forgot to sign the form. We were told about this 10 days after the blood was taken. Utterly disgraceful! Ok, we were convinced that it would not happen again and my wife gave the blood sample for the 2nd time. 10 days later when inquiring about the results, we are told that they dont have the test results. Upon inquiry, we were told that the blood was clot when sending to the lab. (which is within the same hospital). That was their cover up story anyway. Now my wife delivered the baby and the baby had her blood taken for some blood results due to complications. Now again the doctor rang in a week time saying they couldn't get the blood results!(in doctors words he said it was either lost, clotted for not sure what happened) This is utterly disgraceful!
