"Record keeping at this trust just keeps getting..."

Over the last 24 months, I have had the opportunity to partake of the Trust services as both a family member and lately as a patient. On each visit, I alerted staff to my concerns about the quality of record keeping, in what is after all a key element in handing over care between staff for patients. During my stay, in November, not only were my records falsified, something I had previously complained about for family members, and for which an official complaint has been placed, but my diagnosis was also changed on my records between wards, from Epiploic Appendagitis, to Appendicitis, two completely different conditions. This is far from satisfactory. My father was re-admitted to PGH just a fortnight after being discharged. His treatment in A&E was good, with the staff being very helpful. However, upon transfer to ward and subsequent discharge, his records were also not to a sufficiently high standard that basic care should require. I tried to obtain a copy of his discharge letter from his first visit in the middle of November, and this seems to been unobtainable from anywhere, not even arriving at his doctors. This time he was discharged on New Years Eve, and his discharge docs are dated 13/12/13. Neither of these documents are therefore suitable to allow my father to claim on his insurance for time taken in hospital. Rest assured I will be placing yet another formal complaint to go with the others you have seen fit to palm off...
