02.11.13 Flexible cystoscopy - Pinderfields Hospital In conversation with the consultant I mentioned that i'd had surgery in 2006 and that I was allergic to penicillan The hospital had no record of this information so the consultant was not able to proceed with the cystoscopy I was advised that upon receipt of this information another appointment would be arranged 22.11.13 Flexible cystoscopy - Pontefract Hospital My gp had provided information regarding my allergies so the consultant was able to carry out the cystoscopy but the trust still had no notes / record of the surgery I underwent in 2006. When I enquired why my notes were missing the nurse assisting said "we are a paperless trust" I didn't think this was a satisfactory answer
