The incompetence and lack of dcare regarding detail.

Husbands medical records out of date by FOUR years. On admission they aasked who our GP is, Our current address is klnown to the hospital, so why are the records regarding GP and our home address so out of date for the discharge summary. (I do know the answer to this and I know whwer the fault lies).

Husband's discharge note is showing an address we lived at four years ago and the GP we used to attend.

It is obvious the FY1 has not read his file, or maybe the letters from the other consultants have disappeared into the ether or even file 13.

During rounds he was told by the consultant to have a good lunch and he could go home.. I arrived unprepared for this. Turns out he did not have lunch either, or breakfast, no pain relief, I mentioned this to one of the nurses and her reply I was not in charge of lunches, so I suggested she gets something for him to eat as it is now 3.15pm; he is now supposed to have pancreatitis but no tests other bloods have been done. I questioned the FY1 attending him to be brushed off with you can talk to your GP about this, Which GP? The one on his discharge notes or the the GP we see now.

This explains a lot. Our current GP has not records of his last admission to Hinchingbrooke either.

I mentioned to FY1 on admission that I had full set of notes from my husbands oncologists, but she was not interested, If she had read them she would have seen what ius on his discharge summary is not the full story and perhaps they would have sent him home still in pain and dosed up on ora-morph, which he should not be taking.

The night before they want to catheterise him, again, becausew of the incomplete notes, or not bothering to read them, they would have realiused because of previous treatment he NOT to be catherised, unless it is an extreme emergency, This was not, it was for the convenience of the nurses to collect amnd measure his urinary output.

There was a urinal on the bed tray, which had been there for so long it had stasrted to leak and when I mentioned I was given the gear to clean it up

What could be improved

More nursing staff.

Attention to detail.

Listen to what they are told by the patient and carer; (at least this is how I was trained).

Make sure medical records are up to date.

Make sure the correct address and phone number are on the records and the correct GP and Practice are also recorded.
