Our father was admitted to a and e and was then transferred to Holly ward where a safeguarding issue had been raised and we were promised that he would not be discharged home untill we had a meeting at the hospital with our concerns. To our horror when we rang the ward to see how he was we were told he had been discharged home, a complete lack of communication between departments and the ward, the family had grave concerns regarding a frail old man and the concerns are ongoing.
