About: Telford referral and quality service (TRAQS)

(as a service user),

Please stop wasting money by sending out these questionnaires. This was the second I have received. Instead, just ask patients to fill in forms online.

Those that care one way or another will do so whichever ways they are being asked. Postage/printing/envelopes are expensive. I enclosed the other form (better, but not necessarily good quality) so that it can be re-issued.

Thank you for taking the time to provide feedback.

I am sorry that you feel we are wasting money by sending out questionnaires. Unfortunately not everyone has access to the internet which means that those without access would be unable to fill in a form online. We are a relatively new service and rely upon feedback from patients to develop the service further.

We appreciate that if you have been referred for more than one reason the questionnaire may seem unnecessary but we are asking you to provide feedback on each individual experience which we are aware can differ greatly depending upon where you are seen.
