"Volunteer drivers not being given information"

When volunteer driver in Derbyshire takes patients from Care Homes in Glossop to appointments at Tameside Hospital, no staff member goes with them, no medical records are sent and the driver is constantly being asked about medication.

Thank you for bringing this to our attention. Our Social Care partners at Derbyshire County Council agree that the sharing of information between health and social care agencies is vital to the provision of seamless care. Having all of the relevant information to hand enables professionals to make informed decisions about the care you require. This issue will be raised with the Glossop care homes.
