(as the patient),

I attended an appointment a Hackenthorpe medical centre for pain I was suffering in my shoulder/ arm/neck, along with 1 or 2 other concerns I had.

The doctor was referring me to a 'lifestyle coach' due to weight problems, a physio for my shoulder/neck muscles and was arranging for me to have a wrist support for carpal tunnel..

Over 3 months on I have yet to receive any of these, it takes a lot for me to visit and talk to a doctor and it has basically put me off bothering again!
