About: Telford referral and quality service (TRAQS)

(as a service user),

Most of this questionnaire is not applicable. I was referred by my optician and a letter then appeared in the post with a date, time and place for my appointment!

Five months later I might add! !

However, it is not clear from your feedback where your letter of appointment originated from and whether TRAQS was involved in any way in making this appointment. As all feedback is anonymous I would appreciate if you would contact TRAQS on 01952 580441 to allow us to investigate the matter further.

Should TRAQS have been responsible for the delay in any way I would apologise in advance but I would like to have the opportunity to clarify the position to ensure that we avoid any similar problems in the future.
