(as a relative),

My wife and to some degree myself as her carer are in the care of CMHT Leeds. Over two weeks ago my wife's care coordinator visited and due to promotion has now moved to a new post after giving an excellent service. Since that time we have had no communication or contact from that service with regard to ongoing care. Once again we are left isolated and feeling abandoned, because this has happened before. In the past two complaints have been submitted, neither acknowledged.

Dear Frostie,

My name is Kim Lacey and I am the service manager for the community mental health team your wife has been receiving care from.

I was initially pleased to hear that you and your wife feel you have had excellent service from the community mental health team, but also really sorry to hear that you have had no further contact since your wife’s care coordinator left her post and that this has happened before.

I can completely understand how this would make you both feel frustrated and let down by us.

I'd like to look into this as quickly as possible, so please may I ask you to contact our Patient Advice and Liaison Service (PALS) who will be keen to help facilitate contact with me.

PALS can be contacted on 0800 0525 790 or at pals.lypft@nhs.net. This will allow us to ensure your wife receives the right care and establish who should have taken over her care.

I can also personally look into why you have not received a response to your previous complaints, as this should most definitely not have happened.
