"Horrific bruise after blood test"

My mother attended pathology for simple blood test last week. The test hurt her quite significantly (doesn't usually hurt much) and I noticed that the phlebotomist seemed to dig in and upwards with the needle and also performed the test lower down the arm. The arm had bruised terribly only ten minutes later and 2 days later is approx 4" X 3" and is dark purple - really alarming to see. Why would the phlebotomist do things differently this time? My mother has had several blood tests lately with no problems at all and no bruises.
