My father recently died of bladder cancer. His treatmentt and surgery were very good at the beginning,but when he was discharged after it becoming terminal the way he eas sent home was a disgrace. His clothes and most of his belonging were left in his locker at his bedside,he had nothing on his feeet ! The palliative care was very challenging. Discharge from hospital for an 84 year old with terminal cancer was undignified and most challenging for me as his carer and the family. I would strongly support a clinical assistant for discharge for people in the future. Not a so called team on discharge who couldn't get anythhing right with my dad's care plan. Iif it hadn't of been for our GP, we would have had an even more traumatic ordeal with his end of life care. This needs so much attention it hasn't helped our family to grieve. The communications about my dad's condition was disgraceful too. After he died I had to visit my gp to get the full picture and support for me. And my family with un-answered questions.
