"Dischsarge of and elderly female with vascular..."

About: Frimley Park Hospital (Frimley)

Anything else?

My aunt was taken to A/E following a fall on july 6th . She is 89 yrs old and lives alone with carers visiting twice daily and has vascular dementia . My daughter on being notified of my aunts fall visited A/E and was told she wasnt ready for discharge . Over an hour later she was requested to take my aunt home but was unable to do so as in London and I live in Devon as the other next of kin and on the 6th was actually in Birmingham . Later we had several calls from my aunt to say she was at home . On enquiry the carers were not notifeid until we rang the emergency number. we rang Frimley Park and A/E were very helpful and stated she had been sent to F3 . the staff there said no one had been discharged and cut us off There is a need for urgent discharge and I know the hospital has time limits, but poor discharges cause re admission. I drove to visit my aunt on the 7th and she is coping in her own world but denies being in hospital . She thinks she went to a WRVS meeting as she did meals on wheels for over 30 years.

Thank you for bringing this matter to our attention. Proper discharge is a vital part of a patient’s care and recovery and we apologise that this does not appear to have been carried out as well as we would expect in the case of your aunt. We have forwarded the information you provided to the relevant teams.
